... No, it hasn't. Even the blood from the inital penetration of the hymen isn't enough to make someone bleed out. Like I said, unless he's using a chainsaw to hump her, it wouldn't be dangerous enough to kill her in the act.

"Side effects" aren't in the definition because you're under the impression there are any, or there has to be any. The act of rape is forcing someone to engage in sexual acts. Sexual acts aren't fatal. Other acts—having no relation to anything sexual, such as using someone by force to do something—have nothing to do with rape, but physical abuse. And even in this physical abuse, there has to be an assumed intent to kill.
