That, or what if while the person is raping the girl, what if he's a rough guy, (it's rape so I imagen he is) and he's pounding the girl in doggy style while holding her head down, (with full force) and the chick's neck snaps. Hell, it's even possible if the woman is a virgin and the guy is huge, there's a possibility that he could tear her insides a good bit and she'd bleed to death. It has been recorded in some rape victims.

Rape can be fatal, depending on the rapist.
